The primary purpose of this position is to serve as a model for effectively engaging the residents and keeping them focused on their treatment plan. As well as to motivate the staff and hold them accountable. The general purpose of your job is to keep children who are clients of OTS safe at all times and to be respectful of the children and their families as valued clients.

Bilingual Therapist

Job description

Provides community-based individual and family therapy to recently migrated children, adolescents, and their families. The Therapist is responsible for delivering culturally competent services that consist of mental health assessments, screening for human trafficking concerns, crisis intervention and support services and psychoeducational support groups for recently migrated youth as they adjust to life in the United States. The Therapist supports the delivery of Option To Success Inc program trauma responsive model by collaborating with case management team to ensure the youth and their caregivers receive comprehensive services and are connected to both formal and informal resources in their community.

Reports To Treatment Director
Outpatient Therapist
This position will have responsibility for providing Clinical Mental Health Services to children, families, and adults in an outpatient community mental health clinic. Responsibilities will include intakes, diagnosis, case management, and mental health treatment services to identified children, families, and adults. This position is fully remote.
Responsibilities:
• Provide intake assessments, diagnosis, and ongoing culturally sensitive treatment using Evidenced Based Practices to children, families and adults.
• Provide therapy groups for individuals.
• Develop clinical treatment plans which focus on client strengths, culture, needs, and goals within the briefest time frame for successful achievement.
• Participate in ongoing Continuous Quality Improvement to assure the services provided demonstrate best practice standards and meet all contractual requirements.
• Actively participate in individual and group supervision, clinical staff meetings, and other multi-disciplinary meetings
• Staff cases which require specialist consultations and psychiatric consultations as required
• Coordinate client treatment with schools, physicians, and other community resources and referrals to community services whenever necessary
• Practice confidentiality per HIPAA in all related program functions with staff and clients
• Establish and maintain working relationships with other community mental health providers, primary care, and other referral sources.
• Provide services and maintain records in a timely manner to meet contract requirements, level of care standards, and Catholic Charities agency guidelines.
• Perform other duties as assigned.
Education
• Master’s degree in social work preferred or master’s degree in other Behavioral Sciences required.
Experience
• Experience working with children, families, and adults in a professional outpatient setting preferred.
• Mental Health Professional (MHP) designation or eligible

Community Support Associate (CSA)

Provides assigned Medicaid eligible clients the services necessary to allow them to live with maximum independence in the community.

The CSA provides intervention in the form of skills training, cueing, or supervision in the client’s treatment plan; medication adherence and recognizing symptoms and side effects; non-clinical but therapeutic behavioral intervention, support, and skills training; assistance in accessing and utilizing community resources; emotional regulation skills; and developing and enhancing interpersonal skills.

Duties and Responsibilities

  • Completes documentation accurately, within the Medicaid mandated time frames including treatment plans, progress notes, discharge summaries, and intake paperwork.
  • Provides intervention services to identified participants and consults with the treatment team to identify the needs and level of support for the client.
  • Attends bi-weekly supervision with clinical director; utilizing case review feedback and following through with instruction/directive provided.
  • Always maintains confidentiality of client information, including during community-based activities in which services are provided.
  • Follows all safety guidelines with clients, including reporting abuse policies.
  • Adhere to time and attendance policies and maintain a record, in good standing, of regular and reliable attendance and punctuality, in order to provide quality and consistent patient care

  • Perform other duties as assigned

Targeted Case Manager

Is responsible for managing the treatment activities offered to the patients, which may include facilitating therapy groups on patient units; complete thorough assessments; assessing patients? needs and compiling a treatment plan within a multidisciplinary team; planning and/or providing care, treatment, and services; collaboration and exchanging of information with the treatment team, external providers, collateral contacts, and/or managed care organizations; and implementation of the discharge planning process including the completion of a thorough risk assessment and arranging for follow-up care and resources. Utilize time constructively and in an organized manner to accomplish assigned tasks. Demonstrate flexibility and assume added tasks when the department needs change. Is receptive to feedback and incorporates feedback into daily practices

You well be required to getting training that we provide to have Tageted Case Management Trainig Prior To Start . You get paid according to the amount if cases you take. It is required you have at least 2 years experience in case management. You would have to be willing to work in multiple locations. You will link people up to services and coordinate services for them. You will also advocate for them.
